Aphasia-A Global Term
Aphasia is an umbrella term that covers the various disorders of symbolization.
Aphasia results from the damage to the language areas of the brain. The three localised language areas of the brain are: Broca's area, Wenicke's area, the Arcuate fasciculus.
The Broca's area, usually present in the dominant hemisphere of the person, is responsible for expressive language. The damage to this are results in:
1. Non-fluent, effortful and slow speech.
2. Telegraphic sentences.
3. Information content produced is just sufficient for communication, eliminates the grammatic aspects.
4. Cannot repeat others's sentences.
5. Reading and writing skills impaired.
6. Comprehension is slightly impaired.
7. Apraxia may be associated.
The Wernicke's area is responsible for perception of verbal and written matter. The damage to this area results in:
1. Major disruption in comprehension fo verbal and written matter.
2. Speech is fluent, but the words of the intended sentences are jumbled up. Jargon or false words are also seen.
3. Writing is similar to verbal behaviour, letter forms are inappropriate.
The Arcuate fasciculus comprises of the fibres that connect the Broca's area and the Wernicke's area. It is responsible for the association of the incoming and outgoing symbolic information. The damage to this area, known as conductive aphasia, results in:
1. Poor repetion.
2. Irrelevant answers to questions.
There are other non-specific areas involved in language expression and comprehension of the brain. The Supramarginal gyrus and the Angular gyrus are concerned with the written form of expression and comprehension, respectively. Although, it is the left hemisphere (dominant hemisphere), which contains the language areas in most humans, the right (non-dominant) hemisphere also has a significant role in language. It involves the integration and processing information, correlating the suprasegmental features with the information and deriving its meaning, spatial orientation and integration to help reading and writing.
The other specific defects that may or may not be present in aphasics are:
1. Agraphia - Inability to write.
2. Alexia - Inability to read.
3. Acalculia - Inability to calculate.
4. Paraphasia - Jumbled up sentences.
5. Jargon - Meaningless words.
6. Perseveration - Involuntary repetitive behaviour.
7. Noise build-up - When perseveration aggravates due to a complex language task.
8. Anomia - Inability to retrieve the necessary word.
9. Slow rise time - Delay in understanding.
10. Visual agnosia - Inability to recognise pictures or known people in photographs.
11. Auditory agnosia - Inability to recognise spoken words or associate the sound with its source (e.g. speaker).
12. Apraxia - Inability to command a part of body to make a voluntary movement.
